Must meet eligibility requirements ?
- A child must be 4 years old on/before September 1, 2016 Birthday must fall between September 02,2012 ? September 01,2013
- Must be either unable to speak and comprehend the English language (Oral proficiency instrument will be administered) OR
- Family income falls within federal guidelines for free & reduced lunch program (or receives Food Stamps) OR
- Homeless (as defined by 42 U.S.C. Section 11434(a) and Section 11302) OR
- Child of active duty military personnel or injured or killed while serving OR
- Foster child (CPS placement)
Bring the following documents with you …
- Child?s birth certificate
- Paystubs for the month of July OR current Food Stamp Verification (If qualifying by income)-needed for everyone 18 yrs. of age or older living in the household
- If unemployed, need termination letter or denial of unemployment benefits
- Department of Defense ID card (if qualifying as military)
- CPS placement paperwork (if qualifying as foster child)
- Proof of residency ( June or July electric or gas bill or leasing agreement)
- Parent/Guardians Driver?s License or State Issued ID card (w/ picture)
